The battery is ok and gives a steady 4 volts. Is it for audio? Maybe you alreadymeasured it but just to make sure, when your problem aises, have you measured if there is still datwsheet on the gate or even voltage on your PSU?

Checking the datasheet is always a good suggestion. Using the transistor to drive the gate with higher voltage solved the problem. So no R1 and T1 from your diagram link?
